The "Passport" Experience

 Thank you for considering a National Health Care Associates facility for your short-term rehabilitation. Founded in 1984, National Health Care Associates has grown into a network of more than a dozen rehabilitation and skilled nursing centers located throughout Connecticut, Long Island, Bronx, and Brooklyn, New York, each sharing the common commitment to the highest level of quality care, a focus on customer service and providing a nurturing environment for both patients and families. National provides care for more than 3,000 patients and residents. 

National’s “Passport Rehabilitation” program was specifically designed to meet the needs of individuals requiring a short-term rehabilitative stay following a hospitalization or an acute medical episode. While on your short-term “trip” with us, please take full advantage of the amenities and services our facilities offer. During your stay, if you find anything requiring our attention, please let us know as soon as possible so that we may make your stay the best it can be.

Receiving your passport 

Upon arrival, each participant of our Passport Rehabilitation program receives a customized journal which they will keep with them throughout their stay with us and beyond. This journal is designed to provide an overview of our program and our facility, as well as specific services and amenities which may be found at the facility. 

Detailed information may found in this journal, specific to your diagnosis, the type of care you will receive and may include additional educational materials on your diagnosis and other literature which you may find helpful, as well as staff listings and key facility contacts. 

The journal is also designed to help you document your “journey” to recovery, with designated areas for daily activities and therapies you participated in, questions for your health care providers and notes to take home with you upon discharge. This journal is an ideal way to monitor your personal progress and keep with you on future visits to your health care providers. 

Getting the most out of your trip

While it may not be home, our goal is to make a “home-like” environment for Passport Rehabilitation program participants. Special amenities have been added to enhance the experience of a short-term stay, giving attention to details that matter most to our participants. Although every National Health Care facility is unique, all Passport Rehabilitation units offer such conveniences as:

         Television with cable service provided

         Telephone with service provided

         Daily newspaper delivery

         Room service and Selective dining menu

         Electric beds 

All Passport Rehabilitation facilities also offer their own unique touch to their distinct units, such as Café style lounges, complimentary meal upon discharge from a local restaurant, the latest DVD and VHS releases for entertainment, and much more.

Services Available during your trip… 

Occupational Therapy is a rehabilitation service that provides treatment centered on helping individuals achieve independence in all facets of their lives. It gives people the “skills for the job of living” necessary for independent and satisfying lives. Services typically include:

            • Activities of Daily Living Retraining for Independence and Safety

            • Seating/Positioning

            • Home Management 

            • Splinting/Orthotics

            • Cognitive and Perceptual Retraining

            • Fine Motor/Muscle Re-education

            • Outpatient Rehabilitation Program

 Physical Therapy is a skilled rehabilitation service centered on the treatment of musculoskeletal and neuromuscular problems that impact an individual's ability to move freely and function as they desire in their daily lives. Services typically include:

            • Therapeutic Exercises (strengthening and conditioning)

            • Functional Mobility (ambulation, gait, balance, wheelchair mobility)

            • Positioning for Bed or Wheelchair

            • Wound care

            • Assessment and Management of Equipment Needs

Speech Language Therapy is a skilled rehabilitation service that provides treatment of deficits in the areas of language, communication, cognitive abilities, and swallowing disorders, typically including:

            • Swallowing Evaluation and Treatment

            • Rehabilitative Dining

            • Language/Communication

            • Cognitive Retraining

            • Aural Rehabilitation (hearing) 

While the main focus of your stay will be your rehabilitative recovery, including the above mentioned therapies, please feel free to take advantage of our entertainment and recreational programs offered, as a needed break to your hard work.

Our entire Passport Rehabilitation staff, which was specially selected for this unit, has undergone extensive customer service and clinical care training on meeting the unique needs and preferences of our short-term patients…our goal is to not only meet your expectations, but to exceed them. 

The Rehabilitation team on your journey dedicates all their skills and knowledge to a single focused objective: improving your functional abilities through intensive individualized therapeutic services. They are the health care experts in helping you to recover and achieve your “best ability to function.”

Medical Services
Our mission is to provide comprehensive medical management to individuals with dementia and Alzheimer's related diseases and to promote opportunities which are individualized to each resident's physical, social, emotional and spiritual needs.

This program represents a unique departure from traditional geriatric long term care. Utilizing specialty selected and educated staff ensures for the provision of a calm, predictable, comforting and safe environment.

45 bed Alzheimer's/Dementia Unit, specializing in the provision of care for both higher functioning, as well as, compromised individuals under the guidance of a program director.

  • Recreation Program (7 days a week and evenings)

Utilizes an Interdisciplinary Professional Team

  • A Board Certified Psychiatrist, as part of the interdisciplinary team, provides direction as needed for medication and behavioral management
  • Promote high degree of individualized and group programming
  • Weekly Alzheimer's Dementia resident care plan meetings
  • Incorporates family in resident care planning
  • Treatment team meeting coordinated by program director in conjunction with resident care plan coordinator

Comprehensive Alzheimer's/Dementia Program

  • Overseen by an Alzheimer's/Dementia Program Director
  • Incorporates consistent schedule of recreational activities (7 days a week and during evening hours)
  • Utilizes a separate day activity area with a home-like environment for residents able to participate in more extensive programs
  • Promotes optimum physical functioning utilizing our rehab team
  • Provides family, staff and general public opportunities to learn about Alzheimer's Disease
  • Guides family and community at monthly Alzheimer's support group
  • Utilizes dedicated social worker to provide individualized support and education
  • Provides respite and permanent placement for Alzheimer's/Dementia population

Admission Process

  • Receive referral indicating a dementia diagnosis
  • Obtain physician recommendation for placement
  • Alzheimer's Program Director conducts on site evaluation or telephone interview
  • Review referral with Admissions Team
  • Family members/caregivers complete a Resident Information Questionnaire, prior to admission
  • Family members/caregivers tour facility prior to admission, when possible
  • Resident admission accepted
